Fernwood Donation Request

Please keep the following in mind when submitting a request:

- Due to amount of donation requests we receive, we are unable to provide membership donations for every request.

- Donations are fulfilled in the form of a one-year dual level membership (value $50) which arrives to the requestor via email. A pdf membership form will be attached to the email. This form should be given to the prize/auction winner, filled out by them, and mailed back to Fernwood for processing.

- Ample time is needed for both donation and membership processing.

- Verbal, emailed, mailed, or in-person requests will not be considered.

- A request must be submitted at least 30 days prior to the date of the event the donation is needed.

- Membership donations are not guaranteed even if a request has been granted in previous years.
